In our article, there are a few tabulated cells which are in violation of this provision. This represents an oversight on our part and an inadvertent failure to comply with the DUA. For this, we apologize. Although we are fully confident that nothing in our article could be used to identify individuals or personal information, the fact remains that our disclosures are in violation of the AHRQ guidelines.
