Response to Letter by Purroy et al
We thank the authors for their interest in our article and for presenting their analysis of the use of the ABCD score in their own population of patients with TIA. The results they present are quite sobering, with the ABCD score offering little predictive value in terms of 7-day stroke risk in their population. Like our study, their data argue for caution before introduction of the ABCD score to clinical practice.
Risk stratification for patients with TIA remains an enormous challenge and an area of desperate clinical need. Hopefully, we will one day possess tools analogous to our cardiology colleagues’ ECG and troponin measurement—rapid, straightforward, widely available tests with which to predict risk. Whether this will involve brain or vascular imaging, measurement of biomarkers, and/or categorization of clinical features remains to be determined.
