Abstract WP353: Implementation of the Stroke Clinical Nurse Leader enhances patient care
Background and Issues: Stroke patients and their caregivers require formalized education to assist in prevention and post-stroke rehabilitation for optimal outcomes.
Purpose: The purpose of this program was to improve the quality of care delivered to our stroke population, enhance the patient experience, and advance our patients and their caregivers’ knowledge regarding strokes and prevention.
Methods: Implementation of the Stroke Clinical Nurse Leader (CNL) enhances lateral integration of care and evidence-based care in our stroke population. The CNL manages clinical outcomes, adherence to clinical guidelines, patient satisfaction, and quality of care. The CNL serves as a mentor and impacts the care environment. The CNL coordinates and facilitates the multiple services and disciplines impacting on the patient assuring the most efficient and effective goal-directed activities are provided at the right time and in partnership with all other disciplines providing care.
Conclusions: Implementing the role of the CNL demonstrated a reduction in length of stay and improvement in quality measures. Our patients and family members are more knowledgable and prepared to cope with their new diagnosis as well as preventative measures for stroke reccurence.
