We thank Dr Li for his comments on our recent post hoc analysis of THERAPY.1 We agree that given the unique imaging selection criterion of thrombus length ≥8 mm, the trial was indeed selective, and the cohort was not reflective of a general large-vessel stroke population. The imaging selection was intended to identify patients who would be more resistant to intravenous tissue-type plasminogen activator.2 We would like to add that the enrollment rate was 0.5 patients per center per month, when taking into account the rolling initiation of sites.
