The article by Feldman et al1 discussing trends and difficulties in recruitment for stroke trials was read with a great deal of interest. Although difficulties of recruitment into stroke trials are multifactorial, there has been a shift from academic to nonacademic, mostly Primary Stroke Centers as the front line of acute stroke care.
